This reminds me of a conversation my uncle started with my dad, he has a toddler that mumbles to herself and the walls. My dad asks why she does that and my uncle says I don't know maybe she talks to the dead. TBF, my sister keeps this up at age 8, and she no longer believes in it she just likes making up the stories. And she created the war at 8, and the civilizations at 5 or 6. But she always had Mr. Nobody, who had a wife woth my name, and a lot of children who are still aloud depsite the fact the my siste rwas forced to execute Mr. and Mrs. Nobody. I remember reading this study, cannot recall who by, that researched the number of children with imaginary friends where their mother had a miscarriage before they had the child with the imaginary friend. So was the miscarried child the friend? Can't remember the conclusion either but it was a very interesting premise. I had an imaginary friend and yes, my mother had a miscarriage before getting pregnant with me. Oh, I have 5 siblings just as you mention only children not that this is in anyway conclusive! 65% of children have imaginary friends and there is a higher incidence in only children or first born.
