In your example, Ann could be a transgender woman, the pronouns used in the sentence would still be correct.

People who “think” they are transgender exist. Men with feminine characteristics, who have lived their lives and presented as women, have always existed. Not really women. Women with masculine characteristics, who have lived their lives and presented as men have always existed. Not really men. Just like mental health disorders have always existed.
