Reminds me of another one: Two military pilots left base in a helicopter. Later, one radios in and says, "We need you to send another helicopter. This one won't fly." Either the engineer who designed the rigging left something critical off the prints, or the riggers assembling it didn't read those prints properly. There is usually a generous margin for safety in this sort of thing.
